Finger holes are cut small on purpose, because leather stretches and the holes open up as the grip breaks in.

For dowel grips, measure from the tip of the middle finger to the bottom of the palm and match the brand chart. Palm guards are often measured to the base of the middle finger instead.
Between sizes, the smaller size is the usual advice, because leather stretches. New grips should feel almost a little small and the hand should not fully straighten.
Yes. Tight holes can be eased with sandpaper wrapped around a pencil, but only a little at a time, because the leather keeps stretching as the gymnast trains.
